Have you ever thought about what would happen if you were unavoidably delayed, taken ill or called away in the middle of the night?
The Carers’ Emergency Card could offer both you and the person you care for peace of mind knowing that a plan could be activated in an emergency.
Complete and return this form to create your emergency plan. The plan holds information about the person that you care for along with details of who you would like to be contacted to care for them in the case of an emergency. It will be stored confidentially at Carers Together and Coast & Country.
How the Carers’ Emergency Card works
Emergency Card + is an optional addition to the scheme. This enables you to arrange a temporary respite package of care in the home of the person you care for (if appropriate) for up to 48 hours until alternative arrangements can be made for them. Please note that this is only available if the person you care for is over the age of 18.

Is there a cost?
Two cards will be issued (usually within 21 days of receipt of your plan); one for yourself and one for the person you care for. Each card will have a telephone number to ring in an emergency and a reference number, unique to you, for people to quote when making contact. The cards will have no personal details on them. If your card is activated, the pre-planned emergency actions you have provided will be followed.
